1. The controller of your personal data is CENTRUM MEDYCZNE MEDYCYNA PRACY WROCŁAW SP. Z O.O. with its registered office in Wrocław (53-234), ul. Grabiszyńska 241C.
2. The Data Controller has designated a Data Protection Officer, who can be contacted electronically at iod@mp.wroclaw.pl or in writing to the aforementioned address of the Data Controller’s registered office with the notation “Data Protection Officer”.
3. Providing personal data is voluntary but necessary for CENTRUM MEDYCZNE MEDYCYNA PRACY WROCŁAW SP. Z O.O. to respond to the inquiry of the person concerned, or to present them with an offer or information regarding the services provided.
4. Personal data may be transferred to the following categories of recipients: entities providing services to the Data Controller necessary to achieve the above-mentioned purposes, including IT partners, entities providing technical or organizational support, and other subcontractors within the scope of customer service and marketing.
5. Personal data will not be transferred to third countries (outside the EEA).

Personal data will be processed until the purpose specified in the consent is achieved or until the consent is withdrawn, whichever occurs first, and thereafter for the limitation period of claims and until the conclusion of civil, enforcement, administrative, and criminal proceedings requiring data processing.
7. You have the right to rectify data, access data, erase data, restrict data processing, and data portability of the data provided to the Data Controller.
8. You have the right to object to data processing based on the legitimate interest of the Data Controller or a third party, for reasons related to your particular situation, as well as to object to data processing for direct marketing purposes, including profiling.
9. With regard to data processed on the basis of consent – you have the right to withdraw consent at any time. The withdrawal of consent does not affect the lawfulness of processing based on consent before its withdrawal.
10. A complaint regarding the processing of personal data may be lodged with the supervisory authority dealing with personal data protection, i.e., the President of the Personal Data Protection Office (UODO).
