- The controller of your personal data is CENTRUM MEDYCZNE MEDYCYNA PRACY WROCŁAW SP. Z O.O. with its registered office in Wrocław (53-234), ul. Grabiszyńska 241C.
- The Data Controller has designated a Data Protection Officer, who can be contacted electronically at iod@mp.wroclaw.pl or in writing to the aforementioned address of the Data Controller’s registered office with the notation “Data Protection Officer”.
- Providing personal data is voluntary but necessary for CENTRUM MEDYCZNE MEDYCYNA PRACY WROCŁAW SP. Z O.O. to respond to the inquiry of the person concerned, or to present them with an offer or information regarding the services provided.
- Personal data may be transferred to the following categories of recipients: entities providing services to the Data Controller necessary to achieve the above-mentioned purposes, including IT partners, entities providing technical or organizational support, and other subcontractors within the scope of customer service and marketing.
- Personal data will not be transferred to third countries (outside the EEA).

- You have the right to object to data processing based on the legitimate interest of the Data Controller or a third party, for reasons related to your particular situation, as well as to object to data processing for direct marketing purposes, including profiling.
- With regard to data processed on the basis of consent – you have the right to withdraw consent at any time. The withdrawal of consent does not affect the lawfulness of processing based on consent before its withdrawal.
- A complaint regarding the processing of personal data may be lodged with the supervisory authority dealing with personal data protection, i.e., the President of the Personal Data Protection Office (UODO).
